Community Support Grants for Disabled Individuals

For individuals with disabilities and their families, the stress and cost of specialized in-home care and support systems can be overwhelming.  Since 2007, New Trier Township has provided support grants to applicants with disabilities that match the following categories:
·       Autism Spectrum Disorders
·       Developmental and/or Cognitive Delay
·       Physical Impairments
·       Multiple Impairments
·       Mental Health Disorders
Township grant dollars permit individuals or their caregivers to secure in-home therapeutic assistance, equipment, respite care, medical assistance, home/vehicle accessibility modifications or family counseling.
The program is open to all Township residents with an eligible disability, with no restrictions on age or income. Past recipients are eligible and encouraged to reapply. Those interested in applying for fiscal year 2017-18 Community Support Grants must do so from now through Friday, January 19, 2018. Applicants are to submit a Medical Certification of Eligibility, signed by the applicant’s medical provider. The Medical Certification is the only form of documentation that will be accepted.
